This is a classic mid-century modern porcelain ketchup decanter designed by the influential American modernist LaGardo Tackett as part of his Forma series.
I was not able to find another like it for sale.
It was produced in 1959 by Schmid International.
The bottle is in excellent condition. The original cork went missing, so I replaced it with a new one. There is a manufacturing flaw — a tiny bump near the base (please refer to last photo). The lower case “tomato ketchup” lettering is in very good condition—has faded a little at both ends. The back stamp is clearly legible.
The best part is that the original foil “Japan” import sticker is actually still present and beautifully intact.
